The care coordinator & scheduler is responsible for managing the care team and scheduling clients with dedicated care staff in a helpful and professional manner. The care coordinator will assist in answering phones, developing plans of care and daily tasks, identifying and assigning dedicated caregivers with clients, training care staff on the plans of care of our clients, oversight and supervising the care delivered by the care team, communications with families and caregivers, and support the leadership team of Touching Hearts at Home.

Key Responsibilities Include:

  • Manages staffing and the scheduling of caregivers and clients
  • New Clients
  • Fill-in Clients
  • Responsible for New Client and Caregiver success process
  • Daily review of caregiver notes reporting changes in condition, issues, and concerns in the daily standup meeting
  • Address change of conditions for clients provide resources, home health/hospice/equipment recommendations.
  • Update client Plan of Care/tasks as new needs are identified and relay to the caregiving team
  • Report and support the Administrator with documentation of QMP reports
  • Schedules and manages lead and on-call care teams
  • Supports recruiter with new employee personal care training/assesses the competency of skills for new caregivers, quarterly training
  • Meet each new caregiver, understand their strengths/personalities, coach and developing of the staff
  • Daily meetings with HR and management
  • Fill-in caregiving as needed
  • Rotating manager on call supporting the on-call supervisor
  • Weekly reporting for management

Qualifications Required:

  • Bachelor's degree and/or at least two years of scheduling experience
  • Background in healthcare or home care for seniors
  • Maintain regular core work hours during business hours of 8a-5p
  • Supportive, patient, and keeping a positive attitude is essential
  • Outgoing, well organized, outside of the box thinker, and problem-solving skills
  • Well versed in technology, phone systems, processes
  • Solid listening and communication skills
  • Ability to keep a positive attitude in stressful situations
  • Goal-oriented and driven

Pay: $38,000.00 - $50,000.00 per year
